Effective the first of the month after your employment starts with approximately 90% of premiums employer-paid for you and your eligible dependents. Our plan is within a PPO network and offers medical and prescription coverage.
Dental and Vision Insurance
Effective the first of the month after your employment starts with 100% of the premiums paid on the behalf of you and your eligible dependents. Both dental and vision are offered within a PPO network.
Basic Life Insurance
Carrizo provides group life insurance up to two times your annual base salary and again, at NO cost to the employee.
Carrizo offers short-term and long-term disability benefits that provide eligible employees with income protection for an injury or illness after a waiting period of 14 days. This is a company-paid premium with a benefit of up to 60% of your weekly earnings in the case of a disability.
Our Accidental Death and Dismemberment benefit is offered to our benefit-eligible employees, at NO cost to the employee.
Carrizo offers a 401(k) savings plan in which you can enroll and defer before-and/or-after tax contributions. The company matches 100% of the first 6% of employee contributions and employees are fully vested after five years.
Flexible Spending Account and Dependent Care Spending Account
These IRS-regulated benefits provide the option of deferring pre-tax dollars towards qualifying expenses. The Flexible Spending Account allows for up to $2,550 to be set aside for qualifying and unreimbursable out-of-pocket medical expenses, and the Dependent Care Spending Account allows for up to $5,000 to be set aside for the care of qualifying dependents.
In addition to paid holidays, Carrizo provides vacation starting at three weeks per year, and a yearly sick leave allowance.
Carrizo provides its employees the opportunity to purchase voluntary benefits at group rates. These benefits include: Pre-Paid Legal Services; Identity Theft Monitoring; and Supplemental Life Insurance for the employee, spouse and dependents. These benefits, if elected, are conveniently set up as payroll deductions.
